Background: Ethical dilemmas are an integral part of medicine. Whether physicians actually feel that they have made ethically problematic treatment decisions or choices in their work is largely unknown. Identifying physicians with ethical problems, and the types of problems and underlying factors, might benefit organisational and educational efforts to help physicians solve ethical dilemmas in a constructive way. We investigated how the frequency and types of ethically difficult treatment decisions vary by specialty.Method: A mail survey of all non-retired Finnish physicians (...) was conducted in 2004. Of those who had made any ethically problematic treatment decisions, the types of decisions and reasons given for these decisions were asked for. Factor analysis was used to investigate clustering of ethically problematic treatment decisions, and logistic regression to investigate the effect of specialty, adjusted for age and gender.Results: Psychiatrists experienced ethically problematic treatment decisions most frequently, followed by pulmonologists, internists and neurologists. Problems were reported least often by pathologists, laboratory physicians and ophthalmologists. Overtreatment was more common than undertreatment in most specialties, with the exception of psychiatrists who emphasised undertreatment and patient rights issues.Conclusion: Physicians of different specialties differ significantly regarding frequency and types of ethically problematic treatment decisions they have made. Psychiatrists differ from all other specialists in reporting more undertreatment and patient rights issues. Experiencing ethically problematic decisions might affect the quality of care and physician well-being in many ways. The findings could be useful for both under- and postgraduate ethics education. In this article the results of a study of psychological readiness of pregnant women to parenthood are presented. Psychological readiness is defined as a structure consisting of three components: the cognitive, emotional, and behavioral, which is consistent with the single theory of psychological processes L. M. Vekkera. It was found that the main component that determines the high level of psychological readiness for motherhood is a cognitive component. The content of the cognitive component includes an understanding of the child as (...) a value. Some results of the research of value orientations of pregnant women in the structure of psychological readiness for parenthood. To identify the value system of pregnant women, the method of ‘Value Orientations‘ by M. Rokich was applied. The analysis of empirical data was performed using factor analysis and U criterion of Mann-Whitney. Respondents were distributed into two age groups: first group of 21-25 years, second group of 26-30 years. All women are nulliparous. The younger age group of pregnant women is characterized by values that are directed at the outside world. Emotional component dominates in the structure of psychological readiness for motherhood. Values of women in the second group are aimed at children. Cognitive and behavioral components dominate in the structure of their psychological readiness for motherhood. Knowledge of the structure of psychological readiness of women to parenthood will help to diagnose disorders of maternal behavior, to design methods of its correction and prevention. (shrink)

I am very pleased to see the response by J S Taylor to my critique of the “organs debate”. He makes some notable and important points, but also some errors to which attention should be drawn.Taylor erroneously attributes to me concern that the organ debate excessively focuses on saving the lives of a few people. My concern was about the narrow framework within which the debate is embedded and that it focuses on the lives of a few privileged people—those who (...) can pay—while largely neglecting the lives of those who cannot. The fact that some attention has been paid to such issues in some journals does not negate the importance of my claim. Moreover, it is not that the question of millions of premature deaths has …. In two chapters of Enn. II.9 [33], Plotinus discusses the Gnostic idea that the creation of the world is due to the ‘decline’ of a principle that he variously calls Soul or Sophia. The identity of Plotinus' Gnostics is notoriously difficult to establish with any degree of precision; I can only note here that the idea of Sophia's ‘decline’ features in a number of extant Gnostic texts, such as those from Nag Hammadi and the Berlin Codex, as a recent survey (...) of the evidence by Poirier has demonstrated. Frank Arntzenius has recently suggested that we should reject the standard view that the instantaneous state in classical mechanics consists of both the position and the velocity. In his view, the velocity as ordinarily defined-as the derivative of position with respect to time-cannot be genuinely instantaneous, and, thus, it should be excluded from the instantaneous state. After reviewing Bertrand Russell's traditional objections to the notion of an instantaneous velocity and suggesting that Russell's concerns can be effectively answered, I argue that (...) Arntzenius' attempt to show that the standard definition of velocity fails to yield an instantaneous property is not compelling either. Although I review several suggestions that Arntzenius makes against the existence of instantaneous velocity, much of his case rests upon an intuition, which has also appeared in the work of David Albert, about what is required of genuine instantaneous states. I argue in part that there is no reason to accept this intuition, and, thus, no reason to revise standard physical practice when it comes to the classical, instantaneous state. (shrink)

In this response to Jeff Pflug’s review of my dissertation Michael Polanyi’s Integrative Philosophy, I note that Pflug focused on my discussion of possible extension of Polanyi’s epistemology; he has also taken my statements on scientific truth out of context. In addition, he ignored the four major elements of the dissertation, thereby not giving the reader a “map” to the meaning and the rationale of the work – an intellectual biography of Polanyi.

The article examines the sensorium and how it is has been divided to argue that touch underlies what we refer to as hearing. It explores Stockhausen's "Helicopter Quartet" as an extended meditation on the mediation of the senses and the foregrounding of touch in the piece through teletechnologies that serve as prosthetic devices for sound, sight and touch.

This long, diffuse, and intermittently quite interesting work is an attempt to explain and justify the process by which the concept of truth has been transformed in modern and contemporary philosophy from a doctrine of adequation via one of the true vision of things to that of the activity of the free subject. The main figures in this process are Descartes, Leibniz, Kant, and Hegel, although considerable space is devoted to Fichte, Schelling, Nietzsche, Husserl, Heidegger, Wittgenstein, and even K. O. (...) Apel and Hans Wagner. Despite the discussion of Wittgenstein and an odd comment or so about Frege, modern philosophy means primarily modern German philosophy; there is, however, some peripheral treatment of contemporary analytical thought. One may perhaps point to three main aspects of the author’s program. The first is his acceptance of the currently fashionable view that semantics is the orientating branch of philosophy. This thesis follows from the author’s acceptance of the historical rejection of Cartesian and Leibnizian intuition in favor of Kantian and Hegelian emphasis upon rationality as categoricity or conceptualism. The second aspect is Simon’s regular attempt to assimilate semantics into the doctrine of reflection. Hence the emphasis upon concepts shifts from that of objects to a second-order reflection upon the nature of conceptual thinking. The third aspect is in many ways the most comprehensive; at least Simon’s book culminates in its articulation. This is the attempt to assimilate the doctrine of reflection into a conception of freedom as a doctrine of both Recht and communication. I found especially valuable the emphasis upon the contribution of the subject to "objective" conceptions of meaning and truth. There are very good pages on the modern doctrine of intuition and the changing role assigned to imagination. The regular emphasis placed by Simon on negation as determination, and hence upon negation of negation as a crucial element even within non-dialectical logic, is also valuable. I was less persuaded by the treatment of intersubjectivity as an expression of a hermeneutics of communication within the context of a doctrine of individual rights. This Fichtean revision of Habermas and Apel may be the latest chapter in German academic philosophy of the traditional stream, but it does not follow even on Simon’s quasi-Hegelian principles that the text is sound. The deepest problem facing the author is to prevent the concept of truth from dissolving altogether into subjective reflection upon the limitations of objective definitions of truth. Stated somewhat differently, the author fails to persuade this reader that truth is actually the consensus of the community. Whatever the "positivist" deficiencies of Descartes and Leibniz, they do provide us with a concept of truth that is not easily mistaken for ideology.—S.R. (shrink)

This paper discusses the views of 17 healthcare practitioners involved with transplantation on the ethics of live liver donations . Donations between emotionally related donor and recipients increased the acceptability of an LLD compared with those between strangers. Most healthcare professionals disapproved of altruistic stranger donations, considering them to entail an unacceptable degree of risk taking. Participants tended to emphasise the need to balance the harms of proceeding against those of not proceeding, rather than calculating the harm-to-benefits ratio of donor (...) versus recipient. Participants’ views suggested that a complex process of negotiation is required, which respects the autonomy of donor, recipient and HCP. Although they considered that, of the three, donor autonomy is of primary importance, they also placed considerable weight on their own autonomy. Our participants suggest that their opinions about acceptable risk taking were more objective than those of the recipient or donor and were therefore given greater weight. However, it was clear that more subjective values were also influential. Processes used in live kidney donation were thought to be a good model for LLD, but our participants stressed that there is a danger that patients may underestimate the risks involved in LLD if it is too closely associated with LKD.
